中药畅瘘方湿敷联合远红外线治疗对血液透析患者自体动静脉内瘘功能的疗效分析

摘要:目的 探讨中药畅瘘方湿敷联合远红外线治疗对维持性血液透析患者自体动静脉内瘘功能的临床效果。方法 采取随机对照试验设计,2024 年 1 月~12 月,选择在本院就诊的维持性血液透析患者为研究对象,总例数 90 例,随机分组,每组 45 例,对照组单纯采取远红外线治疗,观察组联合运用远红外线与中药畅瘘方湿敷,比较两组患者的内瘘血流量、瘘口内径、穿刺疼痛程度、并发症发生率及治疗满意度。结果 内瘘血流量比较,观察组改善效果较优(P<0.05),瘘口内径比较,两组无显著差异(P>0.05);穿刺疼痛比较,观察组较低(P<0.05),并发症比较,观察组较低(P<0.05);治疗满意度比较,观察组较高(P<0.05)。结论 维持性血液透析在远红外线治疗基础上协同运用中药畅瘘方湿敷可进一步改善自体动静脉内瘘早期功能,增加穿刺段内径并减轻穿刺后疼痛,对自体动静脉内瘘并发症具有一定的预防效果。

Abstract: Objective Exploring the clinical effect of traditional Chinese medicine Changfu formula combined with far-infrared
therapy on maintaining arteriovenous fistula function in patients undergoing hemodialysis. Methods Adopting a randomized
controlled trial design, from January to December 2024, a total of 90 maintenance hemodialysis patients who visited our hospital were selected as the research subjects. They were randomly divided into two groups, with 45 patients in each group. The control group received only far-infrared treatment, while the observation group received a combination of far-infrared and traditional Chinese medicine Chang Fei Fang wet compress. The internal fistula blood flow, fistula diameter, puncture pain level, incidence of complications, and treatment satisfaction of the two groups of patients were compared. Results Comparison of blood flowin the internal fistula showed that the observation group had a better improvement effect (P<0.05), while there was no significant difference in the inner diameter of the fistula between the two groups (P>0.05); Comparison of puncture pain showed that the observation group had a lower level (P<0.05), and comparison of complications showed that the observation group had a lower level (P<0.05); The satisfaction with treatment was higher in the observation group (P<0.05). Conclusion The combination of maintenance hemodialysis with far-infrared therapy and the use of traditional Chinese medicine Changfu Fang wet compress canfurther improve the early function of autogenous arteriovenous fistula, increase the inner diameter of the uncture segment, andreduce post puncture pain, which has a certain preventive effect on complications of autogenous arteriovenous fistula.
